Forms for Civil Filings
Some forms are available through the Clerk's Forms page. Many kits are available at book stores and stationary stores. Also, some forms are available through the Law Library Resource Center website.
If you choose to prepare your own documents, your documents must conform to the Arizona Rules of Court and Arizona Revised Statutes. Both books are available at your local library and at book stores. You may file in the Justice Court if the monetary amount is from $2,500 to $10,000. Contact Justice Court Administration at (602) 506-1337 for further information. Anything over $10,000 must be filed in Superior Court.
Responding to a Summons
If you have been served with a summons and complaint or petition and you wish to respond, you must do so in writing. There are time limitations for responding to a complaint or petition.

Liens
To create a lien using a judgment from a Justice Court, the County Recorder’s Office requires a certified copy of the judgment after it has been filed with the Clerk of Superior Court.
To file your judgment with the Clerk’s Office, you must provide us with a certified copy of your Justice Court judgment and civil cover sheet.
Filing an Out-of-State or Foreign Judgment
Instructions and forms for filing an out-of-state or foreign judgment are available on the Clerk's Forms page. You will also need a Civil Cover Sheet. Detainers
Before filing a forcible detainer with the Clerk, a seven day notice of eviction is required. Neither the Clerk’s Office nor the Superior Court provides any forms for forcible detainers.
Filing For a Name Change
The Law Library Resource Center can provide you the name change forms for all Superior Court locations. You will also need a Civil Cover Sheet, which is available on the Clerk's Forms page.
Filing a Petition for Name Change for a minor is made by the parent or guardian.
Appealing a Civil Judgment
If you wish to appeal the civil judgment you received in the Superior Court to the Court of Appeals, the party appealing must file a written Notice of Appeal with the Clerk’s Office in the Superior Court. A filing fee is required. A cost bond is required to be posted with the Clerk’s Office as well as a bond account charge. There are time restrictions which apply to filing an appeal.
When appealing a judgment from the Justice or City Court, your Notice of Appeal must be filed in the initiating court. After the appeal is filed, the fee paid and bond posted, if necessary, the initiating court will transfer your case to the Superior Court, and the Clerk’s Office will notify the parties of any fees required within one week.
