Family Support Services/ Address and Hours

The Family Support Services Office is located at 201 W. Jefferson in Phoenix and 222 E. Javelina. The office hours are 8 a.m. to 5 p.m. Monday through Friday. The office phone number is (602)37-CLERK, or (602)372-5375. The switchboard hours are 8 a.m. to 5p.m. If you call the office after business hours, recorded information is available.

For free assistance

For free assistance in enforcement of your child support order or assistance in establishing a court order for child support, you may file an application with the Child Support Enforcement Administration of the Department of Economic Security. The phone number is (602) 252-4045. Please request the Applications Department.

What is available?
The Clerk’s Office has two different payment history reports available - an Official Payment History and Online Summary Payment History.

What is an Official Payment History?
The Official Payment History is a detailed report containing several pages that is often used by the Court and other entities requiring a certified document. This is a complete history from the first payment forward.

How to obtain an Official Payment History
To obtain an Official Payment History, you may either come to the Clerk’s Office in person at 201W. Jefferson St. in Phoenix or 222 E. Javelina Dr. in Mesa, or write to: Clerk of Superior Court / Family Support Services / 201 W. Jefferson / Phoenix, AZ / 85003.

What is the cost?
When visiting the office in person, the fee for an Official Payment History is $30.00. For written requests the fee for a mailed certified copy of your payment history is $30.00 and $7.00 for postage and handling.

Written requests should include:

  • The payor and payee names
  • Case number
  • The appropriate fees.

What is an Online Summary Payment History and How to Obtain a Copy

The Summary Payment History is available online and is a summary of payments listing the monthly amounts paid during the current year and the previous year. This summary is a short version of the Official Payment History and will not be certified.

Family Support Services / Change of Name or Address

What you need to do

If you want to process a change of name and/or address for our payment processing records, you will need to either come in person to the Clerk’s Office at 201 W. Jefferson in Phoenix, or 222 E. Javelina in Mesa, or write to the: Clerk of Superior Court / Support File Maintenance Section/ 201 W. Jefferson / Phoenix, AZ. / 85003 or send a FAX to 602.506.1937.

What you need to list for an address change
For a change of address, please list your case number, name, old address and new address, a photcopy of your Drivers License, both parties names, daytime phone number, and effective date of move. Please date and sign the written information.

For a name change, you must include your case number, both parties names as shown on your divorce, your new name, a photcopy of your Drivers License, both parties names, daytime phone number, and effective date of change. Please date and sign the written information.

What is the cost?
There is no cost for these services.

How long it will take
All name and address changes may take up to three weeks to process.
