Request for Accommodations by Persons with Disabilities

The Americans with Disabilities Act Accommodation Request Form can be completed online or printed in PDF format.

If you need help completing this form, please contact the ADA Coordinator for assistance. Alternative means of submitting an accommodation request, such as by personal interview or a tape recording, is available to qualified individuals with disabilities on request.

After receiving the form, the ADA Coordinator will respond to the requested accommodation as soon as reasonably possible.

Grievance Procedure

If you disagree with the response to your request, you may use the Grievance Procedure described in the Clerk's "Notice of Access Information for Persons with Disabilities Policy." Grievances must be filed within sixty (60) days of the alleged discrimination. You may also use any other remedy allowed under federal or state law by filing a complaint with the appropriate federal or state agency.
